Dear Sir/Madam,
I would like to formally inform you that I am cancelling my membership from White Club. I have been transferred by my company to Texas so that I cannot have the benefits of the club.
With this written notice I request the cancellation of my membership, as per the procedure of club. I would appreciate you sending me written confirmation within 30 days that the cancellation has been put into effect. Please refund the unused portion of my payments, as early as possible.
