Dear Ms. Clayton,

Re:Applying for sick leave

This letter is to inform you that I will be taking two weeks of sick leave from

March 5 to March 19, as I am currently unable to come to work due to influenza.

My doctor has advised me to take rest, to get faster recovery and avoid spreading the illness in the office. I have attached all the medical certifications that need to avail sick leave as per our company policy.

I apologize for any inconvenience caused by this leave and look forward to return to the office as usual once I have sufficiently recovered.

I will complete my current projects from my home when possible and send them to the office by mail. Once I return to the office on March 19, I plan to put forth as much effort as possible to equate the lost time.
