I have just opened a Life Insurance policy in your LIC, Edinburg Branch on April 30, 2020. The policy number is 112 233 445. I would like to kindly bring to your notice that my name has been misspelt in the policy. Therefore, I formally request you to make the necessary spelling correction. Necessary documents are attached for the smooth functioning of the procedure.
