Dear Hayner,

I have been a regular physician of Mr. Karan’s family for over 15 years. I am writing this referral letter in reference to their transfer to a new state due to their work issues and thus, it is impractical for them consult me anymore.

They do not have any family history of congenital diseases. They are a healthy family, although Mrs. Karan gets seasonal cold and have an acute depression and anxiety disorder. I will email you all of their health records as soon as possible.
