Dear Mary Kate,

I wanted to inform you that I am leaving my position at YYYY Company and will be starting a new position at MJM Company in two weeks time. I wanted to let you know that it was good working here and I admire having had the opportunity to work with you. Thank you for your support, guidance and encouragement that you have provided me during my tenure at ABC.

I have loved working on group projects with you, and enjoyed having lunch with you in the break room. I will miss you and this great work environment. I would appreciate any continued advice you could provide, as I am starting the next phase of my career.
